Scouting America Medical Form - Parts A B and C
Note Parts A & B need to be filled out by parents and required for all Scouts. Part C is required for summer camp or any camp over 72 hours. It has to be filled out by a Medical professional (Doctor, PA, etc.) and is good for one year.
